Key Finding

Real acupuncture significantly improved lumbar extensor muscle endurance and reduced blood lactate and ammonia levels in women with chronic nonspecific low back pain, though effects lasted only approximately nine minutes.

What This Means For You

Researchers conducted a study to determine whether acupuncture can improve back muscle endurance in women suffering from chronic nonspecific low back pain (CNLBP). Thirty female patients participated in a crossover trial where each received both real acupuncture and sham (placebo) acupuncture in random order, with a one-week break between treatments. The study measured several indicators of muscle performance, including isokinetic parameters that assess muscle strength and endurance, surface electromyography (sEMG) readings that track muscle electrical activity, and blood levels of lactate and ammonia—substances that accumulate during muscle fatigue.

The results showed that real acupuncture significantly improved lower back muscle endurance compared to sham acupuncture. Patients who received real acupuncture demonstrated better performance on all measured parameters: their lumbar extensor muscles showed improved endurance, their erector spinae muscles (the long muscles running along the spine) displayed healthier electrical activity patterns, and their blood showed lower levels of lactate and ammonia. These improvements suggest that acupuncture enhances blood circulation, helping to clear metabolic waste products that build up during physical activity and contribute to muscle fatigue.

However, the beneficial effects were relatively short-lived, lasting approximately nine minutes after treatment. For patients with chronic low back pain, this study provides evidence that acupuncture can temporarily enhance back muscle function and reduce fatigue-related metabolites. While the duration of effect is brief, regular acupuncture sessions might provide cumulative benefits for managing chronic pain and improving functional capacity. If you're considering acupuncture for chronic low back pain, seek treatment from a licensed acupuncturist with experience in musculoskeletal conditions.

Clinical Notes for Practitioners

This single-blind randomized crossover trial evaluated acupuncture's effects on back muscle endurance in 30 women with chronic nonspecific low back pain (CNLBP). Participants received both real acupuncture (RA) and sham acupuncture (SA) in counterbalanced order with a one-week washout period. Two-way repeated ANOVA revealed significant group × time interactions for isokinetic parameters of lumbar extensors, surface electromyography (sEMG) values of erector spinae, and blood lactate and ammonia levels (all p<0.05). Real acupuncture demonstrated statistically significant improvements across all measured parameters compared to sham treatment. The physiological mechanism appears related to enhanced blood circulation and improved clearance of exercise-induced metabolic byproducts. Notably, the therapeutic effect duration was approximately nine minutes post-treatment. Clinical takeaway: Real acupuncture provides measurable but time-limited improvements in lumbar extensor endurance and metabolic markers in CNLBP patients, supporting its use as an adjunct therapy for functional restoration, though frequent treatment may be necessary for sustained benefit.
