Clinical Notes for Practitioners

This randomized controlled trial compared fire needling to conventional filiform needling for knee osteoarthritis in 49 patients (25 fire needling, 24 control) with one-year follow-up. Both groups received treatment at GB34, EX-LE4, ST35, SP10, ST34, and 1-2 ashi points, three times weekly for two weeks. Fire needling demonstrated superior outcomes across multiple WOMAC parameters. Post-treatment, the fire needling group showed significantly lower pain, stiffness, and total WOMAC scores (P<0.05). At six-month follow-up, fire needling patients had lower scores across all WOMAC domains with 76.0% total effective rate versus 41.7% for conventional needling (P<0.05). At one-year follow-up, fire needling maintained superiority in stiffness, daily activities difficulty, and total WOMAC scores with 64.0% effectiveness versus 33.3% (P<0.05). No adverse events occurred in either group. Fire needling shows clinically significant long-term efficacy for knee osteoarthritis, with sustained improvements exceeding conventional acupuncture through one year post-treatment.
