Researchers in China studied whether timing acupuncture treatments to match different phases of the menstrual cycle could improve fertility outcomes for women with diminished ovarian reserve (DOR) undergoing in vitro fertilization (IVF). The study involved 63 women, with 30 receiving acupuncture treatments twice weekly for approximately 15 sessions before their IVF cycle, while 33 received standard IVF care without acupuncture.
The acupuncture protocol used different point combinations based on menstrual cycle phase. During menstruation, practitioners needled points on the lower back. After menstruation, they focused on kidney and spleen meridian points. Around ovulation, they treated points on the lower abdomen and legs. Before the next period, they used points to promote smooth energy flow.
Women who received acupuncture showed significant improvements compared to those who didn't. They had higher estradiol levels, more follicles available for egg retrieval, better quality embryos, and higher fertilization rates. Most importantly, the acupuncture group had substantially higher pregnancy rates and embryo implantation rates, while fewer cycles had to be cancelled due to poor response.
For women with diminished ovarian reserve considering IVF, this study suggests that receiving acupuncture treatments timed to the menstrual cycle in the months before egg retrieval may improve both ovarian function and the chances of achieving pregnancy. The treatments appeared to help the ovaries respond better to fertility medications and produce higher quality eggs. This randomized controlled trial examined menstrual cycle-phase acupuncture in 63 women with diminished ovarian reserve undergoing IVF/ICSI. The treatment group (n=30) received approximately 15 biweekly acupuncture sessions using phase-specific protocols: Shiqizhui (EX-B8) and Mingmen (GV4) during menses; Shenshu (BL23), Geshu (BL17), Sanyinjiao (SP6), and Taixi (KI3) post-menses; Qihai (CV6), Guanyuan (CV4), Zigong (EX-CA1), and Zusanli (ST36) during ovulation; and Qihai (CV6), Guanyuan (CV4), Yanglingquan (GB34), and Taichong (LR3) pre-menses. Controls (n=33) received IVF only.
Compared to baseline and controls, the acupuncture group showed statistically significant improvements (P<0.05) in estradiol, antral follicle count, oocytes retrieved, fertilization numbers, and high-quality embryo counts. Clinical outcomes were substantially better: higher implantation rates, clinical pregnancy rates (both P<0.01), and lower cycle cancellation rates (P<0.01). This protocol demonstrates potential for improving ovarian reserve markers and IVF outcomes in DOR patients through targeted, phase-specific acupuncture interventions.
