Acupuncture plus wet cupping therapy for post stroke depression: a randomized controlled trial.

The study included 130 patients with mild-to-moderate depression following a stroke. Half received acupuncture combined with wet cupping therapy, while the other half received acupuncture alone. Both groups were treated for four weeks and followed for an additional eight weeks.

Patients who received the combination treatment showed significantly greater improvements than those who received acupuncture alone. They experienced larger reductions in depression symptoms as measured by multiple standardized scales. The combination therapy group also showed better improvement in neurological function related to their stroke recovery.

Interestingly, blood tests revealed that the combination treatment appeared to positively affect stress hormones and inflammation markers in the body. Patients receiving both therapies had more favorable changes in cortisol and other hormones related to the body's stress response system, as well as reductions in inflammatory markers that may contribute to depression. Both treatments were safe, with no serious side effects reported.

While these results are promising, it's important to note that this study compared two active treatments rather than comparing them to no treatment or placebo. The findings suggest that adding wet cupping to acupuncture may provide additional benefits for post-stroke depression beyond acupuncture alone, though more research is needed to fully understand the effectiveness of these approaches. Clinical Notes for Practitioners

This RCT evaluated acupuncture plus wet cupping versus acupuncture monotherapy for post-stroke depression in 130 patients with mild-to-moderate symptoms. The combination therapy group (n=65) demonstrated statistically significant superiority across all outcome measures at 4-week post-treatment and 8-week follow-up assessments.

Primary outcome showed greater HAMD-24 score reductions in the combination group (P<0.01). Secondary outcomes including SDS, NIHSS, and MADRS scores all favored combination therapy (P<0.01). Mechanistic biomarkers revealed significant regulation of HPA axis function (cortisol, ACTH) and inflammatory profiles, with reductions in pro-inflammatory cytokines (IL-1β, IL-2, IL-6, IL-8, IL-17, TNF-α) and increased anti-inflammatory IL-10 in the combination group (P<0.01).

Of 130 randomized participants, 118 completed the trial. No serious adverse events were reported. Clinical takeaway: While this study demonstrates incremental benefit of adding wet cupping to acupuncture protocols for PSD, the lack of placebo or standard-care control limits conclusions about absolute efficacy. The neuroendocrine and inflammatory biomarker changes suggest plausible mechanisms warranting further investigation.
