Acupuncture combined with gonadotropin-releasing hormone agonists improves endometrial receptivity and pregnancy outcome in patients with recurrent implantation failure of in vitro fertilization-embryo transfer.

Researchers studied 164 women who had previously experienced recurrent implantation failure (RIF), meaning their embryos had failed to successfully implant during IVF more than once. The women were split into two groups. One group received the standard hormone replacement therapy (HRT) used to prepare the uterine lining before a frozen embryo transfer. The other group received HRT plus acupuncture and a medication called a gonadotropin-releasing hormone agonist (GnRHa), which helps regulate reproductive hormones.

The results showed several important improvements in the acupuncture combination group. Their uterine lining (endometrium) was thicker on the day before embryo transfer — a key sign that the uterus is better prepared to receive an embryo. More women in this group also had a healthy, triple-layer pattern to their uterine lining, and blood flow to the uterus was significantly improved. Most importantly, both the clinical pregnancy rate and the embryo implantation rate were higher in the acupuncture group compared to the standard treatment group.

This study suggests that for women facing the heartbreak of repeated IVF failure, acupuncture may offer a meaningful boost by improving the uterine environment and increasing the likelihood of a successful pregnancy.

Clinical Notes for Practitioners

This prospective controlled study (n=164) investigated the adjunctive effect of acupuncture combined with GnRHa on endometrial receptivity in patients with recurrent implantation failure (RIF) undergoing frozen-thawed embryo transfer (FET). Patients were divided equally into a control group receiving standard HRT for endometrial preparation and a study group receiving acupuncture plus GnRHa plus HRT. Primary outcomes included endometrial thickness (EMT), endometrial morphological classification (A+B type), submucosal uterine blood flow classification, clinical pregnancy rate, embryo implantation rate, and early abortion rate. The study group demonstrated statistically greater EMT one day prior to transfer, a higher proportion of triple-line (A+B type) endometrial pattern, and a favorable shift in submucosal blood flow classification toward Type III on the day of endometrial transformation. Both clinical pregnancy rate and embryo implantation rate were significantly higher in the study group. These findings support acupuncture as a clinically relevant adjunct to GnRHa and HRT protocols for improving endometrial receptivity and IVF outcomes in RIF patients.
